*Print and complete the application for enrollment. Necessary documents to be included with the application:
*copy of immunization record
*copy of birth certificate
*previous school records, if any
*non-refundable registration fee of $50.
*Please return all forms to the CFS office. We accept applications year-round.
*Upon receipt of a complete application it is reviewed by the admissions committee. The committee may require additional references or information as needed to assess the student.
